Introduction: Impacted Third molar surgery is often associated with postoperative complications such as pain, swelling and trismus and dry socket. As the flap design is one of the factors that affect the severity of these effects, this study sought to evaluate the influence of modified triangular flap on dry socket compared to the envelop flap in patients referring to the department of oral &maxillofacial surgery at yazd dental school in 2013. Methods: In this double-blind prospective trial, 30 patients were examined who were candidates for extraction of bilaterally impacted lower third molar with the same difficultly index referring to the department of oral &maxillofacial surgery at yazd dental school in 2013. The influence of modified triangular flap and envelop flap on dry socket was investigated seven days after the surgery. In order to analyze the difference between the two groups statistically, SPSS software (version, 17) was utilized applying fisher exact and Man-Witney tests. Results: This study was examined on 15 females (50%) and 15 males (50%) with an average age of 24.46±3.97. Difficulty index of the teeth was 4-7 and frequency of dry socket for envelop flap was 16.7% and for modified triangular flap it was reported 3.3%, though this difference was not statistically significant(p-value>0.05). Conclusion:The findings of the present study demonstrated that the modified triangular flap design was associated with a lower incidence of dry socket.
